New Patient Registration

The Practice is open to all patients living within the practice area. Patients wishing to register with The Practice for the first time will be given a registration form and two questionnaires (patient and carer) for completion.

Two forms of identity must be provided –

  • a valid passport or photo driving licence
  • a utility bill for proof of address

Newly registered patients will be offered the opportunity to attend for a health check with a member of the practice team.

All patients are now registered with the Practice rather than a specific doctor. If you wish to specify a preference to normally see a specific doctor please contact reception who will endeavour to accommodate your request.

The practice area comprises the whole of the Dulais Valley and the Upper part of Swansea Valley, including the Villages of Crynant, Seven Sisters, Onllwyn, Dyffryn Cellwen, Coelbren and Abercrave.
